Permission to Ride with an Adult Leader

Do you give the adult youth leaders permission to pick-up/drop-off your teen for youth related activities/events this year?

Permission to Participate in Youth Activities

This includes, but is not limited to indoor and outdoor games and activities that may incur injury (paintball, frisbee, tag, etc.), events like Pumpkin Chunkin, the State Fair, Paintball, and traveling to those events with a background checked Youth Leader

Release from Liability

I release the youth leaders, home owners (when an event it in house), and Cornerstone Church from any injury or medical event that arises from my teen's transportation and participation in youth group this year. By selecting "yes" I am adding my digital signature to this form.

Dear parents,

During the school year our youth group meets on Wednesdays for free meals from 5:30-6:15pm, and what we call “Youth Group” from 6:15-7:45pm. During the summer, instead of regular Wed Nights we’ve a number of events. Let us know if you’d like to get on the list to get the details about those events.

Our youth group also meets on Sundays, before the worship service, at 9:30am in the Fellowship Hall with Pastor Tim this Fall.

Please reach out to Pastor Joshua or Pastor Tim with any questions or concerns. There is no “youth pastor,” but we have two pastor-dads who love spending time with teens and seeing families grow spiritually.
